  • Rare sperm cryopreservation: a total of 7 straws were preserved;
  • Single sperm cryopreservation: a total of 40 individual sperm were stored, meeting the needs for subsequent ICSI.

Female Partner

The female partner is 38 years old, in the advanced maternal age category.

Systematic examination showed that her baseline levels of AMH, FSH, LH, and other indicators were generally favorable, with adequate ovarian reserve. Ultrasound examination revealed a moderate antral follicle count (AFC), suggesting good potential for ovarian stimulation.

However, the physician noted that with increasing age, egg quality decline and the risk of chromosomal abnormalities gradually rise, necessitating a more precise stimulation protocol.

Based on her condition, Danruo Reproductive Medical Center in Thailand ultimately designed a personalized PPOS (progestin-primed ovarian stimulation) protocol to maintain egg quality while minimizing the risk of ovarian hyperstimulation syndrome (OHSS).

Stimulation and Egg Retrieval Results Exceeded Expectations

After the full stimulation cycle, satisfactory results were achieved:

  • 21 oocytes were successfully retrieved;
  • 15 of these were mature MII oocytes;
  • 6 were immature oocytes.

Subsequently, all 15 mature oocytes underwent ICSI.

The final results were:

  • 13 oocytes showed normal fertilization (2PN);
  • 2 oocytes showed 1PN fertilization.

Meanwhile, the remaining 6 immature oocytes underwent IVM (in vitro maturation) culture. Of these, 5 matured successfully and all fertilized normally (2PN). After extended culture, 3 formed Day 5 blastocysts.

In total, the full cycle produced 14 high-quality blastocysts, providing ample options for subsequent embryo screening and transfer.
